Make the Most of Your Visit with Your Medical Provider

 

  1. Always have the following information ready to provide to us when you come to the office or call the office:
    • Full Name
    • Date of Birth
    • The best contact phone number
    • Most current insurance information
    • Most current address
    • Emergency contact
    • Reason for your visit
  2. If you are a new patient, please arrive 15 minutes before your appointment.
  3. It is your responsibility to provide this office with any change of personal information, for example, change in your insurance, new mailing address, etc.
  4. If you are taking medication or vitamin supplements, please let your medical provider know.
  5. Keep a record of your health concerns and specific symptoms.
  6. If you are going to be over 15 minutes late for your appointment, you may be rescheduled to another day to see your doctor.
  7. We reserve the right to discharge a patient from this practice with a 30-day notice to patient.
  8. If you have not seen a doctor from this office within the last 3 months and need a prescription for a controlled substance you MUST make an appointment. No triplicate prescriptions will be given without seeing your doctor.

What is an MD? (Doctor of Medicine)

MD’s are trained in delivering medical care. They are fully qualified physicians licensed to practice medical care and prescribe medicine in the United States. Training programs include four years of medical school, three year internship and special medical training in the doctor’s chosen specialty.

What is a DO? (Doctor of Osteopathic Medicine)

Doctors of Osteopathic Medicine, or DOs, are fully licensed physicians who practice in all areas of medicine. Emphasizing a whole-person approach to treatment and care, DOs are trained to listen and partner with their patients to help them get healthy and stay well.
